How to Add and Edit Surveys

This article discusses how to create surveys. To learn more about recording survey responses, please see this article.

Click Settings on the left-hand navigation menu.

Click Surveys

A list of previously created surveys will be listed.

Click Add Survey Form + to build a new survey.

On the Survey Form Properties form, enter the survey name, organizations allowed to respond, and the administration name (e.g., Spring 2026). If the survey will be offered more than once, click the + Add administration link to add another.

Click Save when completed. Your new survey will appear in the list.

Click the pencil icon to start adding elements and questions to your survey.

The Gear icon enables you to edit the survey name and administrations.

The Copy icon will make a copy of the survey.

Elements

Elements are non-question parts of a survey that help organize sections or provide information. To add one, click it in the right menu to open a modal. After editing, click Done to add the element to the survey's bottom.

Heading Elements

Clicking Heading 1 or Heading 2 opens a modal window where you can type in a custom heading in the Display text field. Heading 1 is a larger font than Heading 2. 

Click Done when completed. The Heading text you entered will be inserted into the survey at the bottom.

 

 

 

 

Paragraph Element

SST RegForm AddElem Paragraph.pngThe paragraph element allows you to enter a text block of up to 2000 characters. 

Clicking the Paragraph element opens a modal window. In the Display text field, enter the text you would like to appear on the survey.

Click Done when completed. The text will be inserted into the survey at the bottom.

 

 

 

 

 

 

Horizontal Line Element

The Horizontal Line element simply inserts a horizontal line into the form. This is useful for dividing up sections to improve survey readability.

Click Done to insert the horizontal line into the survey at the bottom.

 

 

 

 

For all elements, use the drag handle icon to move the element to where you want it on the survey.

You can use the gear icon to open an edit modal window where you can change information about the element. 

You can click the trash can icon to remove the element from the survey.

Questions

To add a question to the survey, click the question type from the list at the right. This will open a modal window where you can modify the question information. 

Each question type has different fields to fill out on the modal that appears. For example, the image below shows a Multi Line Text question modal. 

All question types allow setting of the question's required status and visibility settings.

If the question should be required for this survey, click the Required Field toggle to adjust its position. The toggle will turn green if the field question be completed before the survey can be submitted.

When do you want the question to show? The options for the Visibility field are "Always show" and "Depends on."

If the question should always show on the survey, choose Always show.

If the question should only show on this survey when a previous question's data is a certain value, choose Depends On. 

Then, using the Show this field when dropdown, choose the field whose visibility depends on. 

The only "dependent" fields listed are Pick List / Dropdown type fields that appear before this field in this section of the survey. 

Use the drag handle icon to move dependent questions above the question on which you want to base visibility. If no questions fields exist, "No available options" will appear in the Show this field when dropdown.

Then, choose the field value in the Has value of a dropdown menu. In this example, the question would only show if the "Adult" field is marked "No."

The options in the dropdown menus for Show this field when and Has a value of will vary based on the type of question selected.

This "Depends on" feature is often used for questions that may have an "Other" value option. If the "Other" value in the question answer is selected, the "depends on" question will appear. 

When you have completed setting up the question with its correct label, required status, order, and visibility, click the Done button. 

The question will be inserted at the bottom of the survey. 

You can move the drag handle icon to the desired place on the survey.

Use the gear icon to change the question's settings in the modal window that appears.

Within the modal, click the red trash can icon remove it from the Survey.  Confirm the removal on the next Are you sure? screen with Yes, delete. If the field has already been used on any submitted application, it cannot be removed from the form; this will be indicated by a grey-colored "Remove...." text and a grey-colored trash can icon.

Once you are done building the survey, click Save Changes.
